Ear infections:-

Infection of middle ear in children ( 1 to 3 years of age ) is called as ear infection in child.

The children are at high risk of getting upper respiratory tract infection. The upper respiratory tract infection can cause ear infection in children, especially in
children.

Causes of Ear infections in children:-

Upper respiratory tract infection,

Excess wax accumulation can cause ear infection.

Low immunity in children, causes ear infections.

Cold and sinus infections,

Throat infections causes ear infections.

Symptoms of ear infections:-

children will complaint pain in ear.

If the children can’t talk, he will try to show discomfort in ear.

child may not get sleep,

Mild to moderate fever,

Lack of activeness,

Complaint increased pain during chewing.

Are main symptoms of ear infections in children.
Treatment of Ear infections in children:-

After diagnosis of ear infection,

Antibiotics such as erythromycin are given to the children.

Provide rest to the baby,

Don’t give solids to eat if pain is severe.

Provide protein rich diet to child, such as pulses, etc. ..

Decubitus ulcer

Bedsores are the skin lesions caused due to prolonged pressure resulting in the death of skin and dermatic tissue. Bedsores are also called as Pressure sores or pressure ulcers. Main cause of pressure sore is sitting or lying in the same position for prolonged time.

Mechanism of Bedsore development

When an individual stays on single position for prolonged time, the pressure is exerted between the body skin and the other surface such as Bed. The pressure cut offs the blood supply to particular area and leads to tissue damage of skin and underlying tissue, resulting in Bedsores, or pressure ulcer.

Bedsores are caused by number of factors such as prolonged pressure, friction, humidity in the skin, old age, wrinkled bed and friction, increased temperature, and bony prominences with less adipose tissue are at risk. Bedsores worsens the life of individuals if proper care is not taken.

Due to illness, when a single position is maintained for long time, pressure exerts between body skin and bed. Increased pressure creates a force which resists blood flow to that area. Decreased blood flow to particular area leads to cell death, so the skin tissue and underlying tissue damage occurs. This is called as bedsores, due prolonged stay on bed, or Pressure sore, because the sore is occurred due to excess pressure.

Causes of Bedsore

  • Various factors causes bedsores, such as
  • Staying on wheel chair or being bed ridden for long time,
  • Fragile skin
  • Reduced blood flow to skin in conditions such as diabetes and vascular disease,
  • Malnourishment leads to less adipose tissue and causes Bedsores,
  • Inability of person to get aware of skin lesions or irritation, in chronic conditions such as stroke, alzheimers disease,
  • Wrinkled skin in old age which undergoes pressure,
  • Urinary incontinence,
  • Bowel incontinence,
  • Poor hygiene and wrinkled bed, etc

Factors causes Bedsore

Pressure :  Prolonged single position or decubitus, compresses the skin and underlying tissue between bony prominences of body and Bed. The pressure is exerted between bed and body skin. Increased pressure reduces blood flow to the area, decreases tissue perfusion, ischemia occurs and causes tissue necrosis i.e. Bedsores.

Sheer force : A force created between body skin, when patient in same position as a deep fascia and skeletal muscle slided down with gravity. Resistance developed by this force, reduces blood flow and leads to tissue necrosis, causes bedsore.

Friction : Friction occurs between body skin and bed while jerking the body on bed causes Bedsores.

Symptoms of Bedsores/ pressure ulcer

Stage 1 : Red patch is found on the skin, when pressed on patch, it doesn’t change to white. Is the starting stage of bedsore.

Stage 2 : Blisters are formed on the skin, later blisters may become open, red irritating area is found.

Stage 3 : sore or lesion is formed on the skin, even the underlying tissue damage occurs.

Stage 4 : Deep ulcer is formed along with damage to muscles and bone. Usually the bony prominences at joints are affected with bedsore or pressure ulcer.

Treatment of Bedsores

Group of health care team provides care to treat Bedsores.

Nurse places a major role in treating Bedsores.

Change the position of patient as the bedsore area get relief.

Dress the wound cleansing with betadine and other antibiotic solutions.

Improve the nutrition of patient,

If needed with doctors order administer the antibiotics.

Prevention of Bedsores

As we know prevention is better than cure, with proper health care Bedsore can be prevented.

Change the position of patient fro every two hours,

Bed should be soft and neat with no wrinkles,

Pressure reducing things such as pillows, sheepskin, foam padding and powders to be used to prevent bedsores.

Range of motion exercises are provided for immobile patients, daily.

Provide orange juice and Vitamin C rich food to enhance healing,

Provide special care for unconscious patients, patients with urinary and bowel incontinence

Gross hematuria : causes, symptoms, treatment

Gross hematuria:-

Gross hematuria, is the presence of plenty of red blood cells in the urine as the red blood cells can be seen in naked eyes. presence of red blood cells in urine is known as Hematuria on medical term.

Gross hematuria is contrast to microscopic hematuria in which blood is visible only under a microscope. Gross hematuria an urinary abnormality, should be diagnosed earlier and treated properly. Gross hematuria may associated with pain or without pain. Infections and stones in the urinary tract are the common cause of Gross hematuria with pain. Painless hematuria also has many causes. Appearance of blood only time in urine may not be a serious disorder. Even a Strenuous exercise, Drugs such as aspirin also causes blood in urine.

Causes of Gross hematuria:-

Beeturia :- blood in urine seen after the ingestion of beetroot is called beeturia. Is not a disorder and is caused due to excretion of betalain reddish pigment.

Red blood cells enters the urine from glomerulus while blood is filtered in the glomerulus apparatus. Abnormality by the glomerulus to filter blood leads to red blood cells in the urine.

* Inflammation of bladder or prostate,
* Rupture in the urethra,
* injury to urinary bladder,
* Presence of polyp or tumor in the urinary tract,
* Malignant tumors,
* Kidney or ureter stones,
* Enlarged prostate,
* Urinary tract infection
* Inherited diseases,
* Injury to kidney, etc

Symptoms of gross hematuria:-

* Visible red blood cells in the urine,
* Red or brown colour urine,
* Painful urination with red blood cells,
* May be associated other disorders.

Treatment of Gross hematuria:-

Treating the underlying cause is the main treatment. If urinary tract infection is present, antibiotics are prescribed. Any abnormality of glomerulus or Kidney diseases should be treated.  ..

Intestinal tuberculosis: symptoms

Intestinal tuberculosis is the infection of intestine by mycobacterium tuberculae, the Tuberculosis causing bacteria.

The clinical manifestations of intestinal tuberculosis are

1. stomach ache,
2. abdominal pain
3. weight loss,
4. Patient may feel presence of mass in right iliac fossa,
5. Obstruction in the right side of intestine,

Intestinal tuberculosis also results in Malabsorption, and lymphatic obstruction.

Barium examination is the main diagnosis, which reveals, irregular intestinal folding, and thickening of intestinal wall.

Anti tuberculin drugs are the treatment option with other medical interventions.

Knee Arthritis : causes , symptoms , treatment

Knee Arthritis:-

Knee Arthritis is the form of Osteoarthritis. Knee Arthritis is also called as degenerative disease or Wear and tear arthritis.

Osteoarthritis causes the degeneration of articular cartilage of knee joint.
The lack of articular cartilage leads to bare bone, will crush each other during mobility and causes pain in the Knee joint.

Causes of Knee Arthritis:-

  • Genetic predisposition causes knee arthritis in number of individuals.
  • People aged above 50 are at risk Knee arthritis, as the aging process causes knee arthritis.
  • Over weight persons or obese will get knee arthritis, because the knee joints need to bear excess weight, and the articular cartilage tears.
  • Meniscus tear in the knee joint causes knee arthritis.
  • Fracture or Trauma to knee joint also causes knee arthritis.

Symptoms of knee arthritis :-

  • Stiffness of knee joint,
  • Pain associated with mobility,
  • Difficult knee movement,
  • Increased pain while standing after prolonged sitting.
  • Swelling of knee joint,
  • Tenderness of knee joint,
  • Sensation of knee joint may give out, are main symptoms of knee arthritis.

Treatment of knee arthritis:-

#  Physiotherapy is the best treatment of knee arthritis.

#  Exercises, especially aquatic exercises are beneficial.

#  Reduction of weight is recommended for over weight persons.

#  Walking aids are used to minimize the weight on knee joint.

#  STEROIDS are given to reduce inflammation.

#  Knee Osteotomy ,

# Knee replacement surgery..these are the treatment for knee arthritis.

kidney stones passing : causes and symptoms

Kidney stones passing :-

Kidney stones are the common urological disorder. Adults are more prone to have kidney disorders. Kidney stones usually diagnosed in the later stages, while diagnosing the cause some other problems. Formation of crystalline in the kidney due to accumulation of salts is known as kidney stones.


Causes of kidney stones:-

Number of factors causes kidney stones. The exact cause of kidney stones is not known. The suspected causes of kidney stones are:

* Genetic predisposition or family history of kidney stones, may cause kidney stone formation.

* Recurrent urinary tract infections may lead to kidney stone formation.

* Cystic disease of kidney,

* Hyperparathyroidism and other metabolic disorders causes kidney stone formation.

* The hereditary diseases such as Renal tubular acidosis favours kidney stone formation.

* Cystinuria and hyperoxaluria are the inherited metabolic disorders causes rarely causes Kidney stones.

* The inherited factor, Hypercalciuria Cause formation of kidney stones in more than half patients.The excess calcium absorption in body is accumulated in the form of crystallines is called kidney stones.

* Drugs which increases calcium absorption in the body, such as calcium antacids, leads to kidney stone formation.

* Diseases such as Gout, the uric acid metabolism disorder in body, also causes formation of kidney stones.

* Hyperuricosuria is another cause of kidney stone formation.

* Calcium oxalate stones are formed in patients have some other disorders such as chronic bowel inflammation, intestinal bypass surgery etc.

* Chocolate, spinach, wheat germ, sweet potatoes are high oxalate containing foods increases the risk of kidney stone formation.


Symptoms of kidney stones:-

The person having kidney stones may not show any symptoms for some weeks to months. If kidney stones are ignored for long time , lead to the symptoms.  After the kidney stones becomes large, they block the passage in the kidney, and obstructs urine flow. Kidney stone may present in single kidney or both. The symptoms of single kidney stones are less, as compared to stones in both kidneys.

* Pain during urination is the main symptom of kidney stones.
* Infection of upper or lower urinary tract.
* Blood cells may come out along with urine.
* Difficulty in passing urine,
* Change in the colour of urine,
* fever, chills, due to urinary tract infection
* Difficulty in holding urine

Frequent urination, discomfort with urination are the main symptoms of kidney stone.

Ringing in the Ear or Tinnitus : causes, symptoms

Ringing in the Ear:-

Perception of ringing sound in the human ear, in the absence of external corresponding sound is called as Ringing in the Ear.

Ringing in the Ear may occur in single ear or both the ears. The ringing sound will be felt in different manner in different individual. Ringing sound may be felt as whistling, ticking, roaring, buzzing, hissing, clicking etc..

Ringing of ear is not a disease, but is a symptom of underlying cause.

Nearly 35 million Americans suffer from the problem of ringing in the ear or some sounds in the Ear.

Tinnitus can be felt at any part of ear i.e. outer ear, middle ear, or inner ear. Some tinnitus is normal, and heard when all external noises are
ceased.

Causes of ringing in ear:-

Ringing in the ear is normal and when waxes obstructs the external ear,the inner sound is heard.

Ear infections,

Fluid in the middle Ear,

Disease of middle ear bones can cause Tinnitus or ringing sound.

After exposure to loud noise, some sounds are heard in ear.

Medications such as Aspirin can cause Tinnitus.

Damage, or deformity of tympanic (ear drum) membrane can cause ringing in the ear.

Appendicitis during pregnancy and Management

Appendicitis during pregnancy:-

One in thousand pregnancies will get appendicitis. Even appendicitis is not difficult to treat in non pregnant state, appendicitis in pregnancy is difficult to manage.

The appendicitis pain is not felt at the exact region  and is difficult to diagnose during pregnancy.

Due to uterus distension, the colon and small bowel is displaced.  The uterine contraction prevents the abscess formation.

So nearly in 20 % pregnant mothers, Appendix is ruptured and causes peritonitis.

Delayed care may lead to pre-mature  labor  or Abortion.

In pregnancy, early appendectomy is indicated.

If appendicitis is diagnosed near term, or labor, Extraperitoneal Caesarean section and appendectomy to be done in order to prevent Peritonitis.

Therapeutic abortion is never indicated for appendicitis during pregnancy.

Tansabdominal drains are placed of needed after surgery.

Early detection of appendicitis during pregnancy helps in preventing complications.

Lung treatments are used to treat lung disorders such as COPD exacerbation.  Lung disorders are the anatomical or physiological deviations of the lungs. Lung disorders need the physicians care as the lung is the vital organ, helps in the gas exchange form blood.

Tongue lesions : sore tongue, causes, treatment

Tongue lesions:-

Lesions occur on the tongue are considered as tongue lesions. Most of tongue lesions are healed without treatment. If tongue lesions are not healed for long time, biopsy is taken and test is conducted to identify any underlying cause.


Causes of tongue lesions:-

* Vitamin deficiencies, are common cause of tongue lesions.
* Malnutrition,
* iron deficiency anaemia,
* pernicious anaemia,
* HIV infection,
* Oral candidiasis,
* Local infection,
* trauma to tongue,
* Side effects of medications,
* beverages or tobacco induced lesion,
* tongue malignancy lesions are the causes of tongue lesions.


Treatment of tongue lesions:-

*  Treat the underlying cause such as vitamin deficiencies.

* Apply analgesic cream on the tongue lesions,

* Systemic analgesics,

* Mouth washes with analgesic effect,

* Avoid mucus irritants such as tobacco, hot foods or drinks,etc.

* Maintain good mouth hygiene and dental hygiene.

* Use mouth rinses such as lidocaine 2% topical solution, etc.
