Mummy asked 3 years ago

I’m working my way back up to 750 and doc told me we had me taking it at night…  does this seem right? i read a previous post about half-life and wonder if i should take half in the morning and half at night instead?

Re: best time to take Keppra
the PI sheet says a steady state is acheived after 2 days of twice-daily dosing.    i take it 4 times a day.

i think you’d be better off splitting it between AM & PM doses.

i just read your signature line.  which one of your meds is supposed to be working on your depression?

Re: best time to take Keppra
A Keppra titration starts at night, then moves am/pm, then adds to the pm, then to the am, etc. Like this (dosages for demonstration purposes only, some people start lower, some people go up in smaller increments):

500mg pm x 1 week

500mg am and 500mg pm x 1 week

500mg am and 1000mg pm x 1 week

1000mg am and 1000mg pm –> patient is at dose.

my pdoc tried doing that with me, but it didn’t work.  taking too much at night made me extremely depressed & didn’t prevent my afternoon/early-evening mania.  adding additional 125mg doses throughout the day worked much better.

Well, so much for “recommended dosing”.

I’m glad you’ve found a plan that works for you.

Although the fact that it depressed you only when you took most of it at night is curious to me: you should have a blood level of the drug in your system at this point, so when you’re taking it shouldn’t be affecting that.  i.e. if your level is high enough to get depressed, you should be depressed, as it has a 7 hour half life –> will be in your system for 14 hours.  Or maybe your system  just metabolizes the drug really fast.
